Six agency social workers provide developmental disabilities case management services to children and adults across a variety of settings, within the family home, individual apartments, adult foster care or intermediate care facilities.

Case Management Functions & Goals

Case management functions include eligibility determination, information and referral, screening, service planning and monitoring, and funding authorization. The goals of case management services are to promote inclusion, self-sufficiency and the highest level of community independence, support social relationships and natural supports, and to maintain a balance between safety and opportunities.

Supporters Role

Supports are also in place to protect people with developmental disabilities from abuse, neglect or exploitation, to support and preserve families, maximize functional independence and promote community integration Some of the services available to consumers are:
